Increase awareness and use of Advanced Directives in all ages groups in the Gorge

Proposals welcome up to $59,000

An area of interest of the CAP is creating the norm of having the conversation about what we wish our care to be if we are not able to speak for ourselves.

The CAP wishes to support a 2-year campaign and community outreach effort that raises awareness regarding the importance having an Advanced Directive in place and having the conversations with family members and clinicians regarding their wishes.

Proposals need to articulate how they will achieve the following objectives:

Reach at last 25% of the adults in Hood River and Wasco Counties and be representative of the diversity in the community.
Be inclusive of residents in both The Dalles, Hood River and the less populated areas of Antelope, Dufur, Maupin, Mosier, Shaniko, Tygh Valley, Wamic, Pine Grove, Pine Hollow, Cascade Locks, Odell, Parkdale, Mt Hood, Rockford and Oak Grove.
Be relevant for all adult age groups and ethnicities.
Motivate those who are 50 years or older to get an Advanced Directive in place
