Community Grant Program awards are designed to address at least two of the following:
- Columbia Gorge Regional Community Health Improvement Plan (CHIP). The collaborative regional CHIP identifies the highest need areas as chosen by the Community Advisory Council.
- One or more of the current Transformation Plan topic deliverables. The Transformation Plan highlights the key areas to transform healthcare delivery for Hood River and Wasco Counties.
- Innovative ideas that improve current or future Quality Incentive Metrics (QIMs). QIMs are a specific set of measures used to track quality of care for Medicaid recipients. QIMs represent a source of monies which supports future Community Grant Program funding.
- Support Workforce Development to support CHIP topics, Transformation Plan or QIM performance.
- Patient Education Materials and/or Patient supports not covered by Medicaid, other Health Plans or CCO Flexible Services.
Funding Amounts and Decision Timelines
Funding can be requested for a minimum of 12 months and a maximum of 36 months and must fit within one of three funding tiers. The timeline for an award decision depends on the funding tier.

- Grant Proposals are due between 8am September 1st, 2017 and 5pm September 22nd, 2017 Pacific Time and must be submitted electronically at https://webportalapp.com/appform/columbiagorgehealthcouncil2017
- Proposals submitted outside of these times will not be considered. Details on electronic submission are in the FAQ section.
- Funds will be available within 4 weeks of PacificSource Community Solutions and Columbia Gorge Health Council having received signed acknowledgment letter and related contract paperwork.
- Funding start dates will begin upon receipt of monies.
- Awardees will be expected to provide updates every 6 months for the duration of the funding period and accept occasional requests for brief discussions.
- Proposals are intended for initial start-up or supporting advancement of a program and not for unending sustaining operations.
- Proposals will be accepted from the following sectors located in the Gorge:
- Nonprofit organizations
- Public and private schools including early learning
- Government or tribal agencies
- For-profit organizations located in the Gorge region
- Preference given to partnerships including at least 2 organizations highlighting cross-sector or cross-county partnerships
- Priority given to enhancing and expanding services – clearly articulating non-duplication
- A clear explanation of a sustainable model for continued funding if applicable
- A clear explanation of how Gorge CCO clients or Gorge CCO service providers will be impacted
- Clearly defined outcomes and measurements
- Clearly defined budget including a 15% match (In-Kind or Cash)
- Be applicable and/or replicable across the Gorge region
- Lead organization must be located in the Gorge region
Complete Applications will include:
- September 2017 Community Grant Application (PDF) or Word version
- Completed Community Grant Focus Area Checklist
- Completed Community Grant Proposal Goals, Activities and Measures (PDF) or Community Grant Proposal Goals, Activities and Measures (Word)
- Completed Community Grant Budget form
- At least 2 letters of support from outside organizations
All applications will be reviewed to determine if minimum application requirements have been met. Only complete applications will be reviewed by the Funding Committee. The Funding Committee is made up of at least 1 member of the CAC, CAP, Board and plus optional United Way Board representative and 1 local representative from each Hospital foundation or equivalent (MCMC, Providence, KVH and Skyline).
Those who vote on the funding committee may not have conflicts of interest with the applying agency. Upon approval by the funding committee, the application must also be approved by:
- Tier 1: The CAC or CAP
- Tier 2: The CAC or CAP and the CGHC Board
- Tier 3: The CAC, CAP and CGHC Board
Funds may not be used for:
- Insurance Enrollment
- Covered Health plan Services
- Preparation, distribution or use of materials to directly support or defeat proposed legislation.

- Can an organization submit more than 1 application? Yes. When submitting more than 1 application, indicate in the narrative how your organization will be able to successfully manage more than 1 application.
- Will Letters of Support provided for other community partners impact an organizations own application submission? Letters of Support written for another applicant will not negatively impact an organization submitting their own application. If an applicant is part of the work team listed in a proposal with a different lead applicant, the applicant should include a brief statement on how they are able to effectively staff more than one proposal.
- Does the Columbia Gorge Health Council (CGHC) foresee fully funding proposals or is there potential the CGHC will reduce the amounts of some requests in order to fund more? The range and number of proposals is unknown at this time. During the review process, CGHC may encounter overlapping or very similar proposals that could be combined, proposals that unintentionally include costs for covered benefits or include costs that can be funded through other means. These circumstances might lead to reductions in proposal requests to size the proposal appropriately. CGHC anticipates a preference to fully fund ‘right-sized’ proposals over partially funding many proposals. The final funding decisions are in the purview of the CGHC Board
- What is meant by Workforce Development and Education Materials and/or supports? Workforce Development includes educational scenarios that may be achieved through traditional coursework, adult education classes and seminars, meeting certification requirements and/or hands-on experience leading to improved capability for the individuals or increased capacity in an organization in a work setting. Workforce Development must be combined with a CHIP topic, a Transformation Plan item or a QIM. Education Materials and/or supports are not limited to a work setting and may be items for an individual’s personal use to advance their health or for use by the community overall.
- Can the Activities Grid be expanded? The Activities Grid can be adjusted to meet the needs of the grant applications. Specifically, applicants can remove any Focus Area rows that are not part of the Grant scope and add space for the Focus Areas that are included in the proposal. Limit the content to 2 pages total and ideally 1 page. Keep font size and margins unchanged from the Word template on the website.
- On the application, question #11 asks for names of team members and their “current trauma-informed practice capability” with “Advanced, Experienced, Beginner, Novice” in brackets. What information is needed? For question #11, the application would have 2 lists – one of the team members and one of the organizations. For the team members, include their name, their role and the organization they are affiliated with. On the organization list, include the names of the organizations only and specify the organization’s current trauma-informed practice capability using one of: Advanced, Experienced, Beginner, or Novice. This is a self-assessment of the capability using the following definitions as general guidance. Advanced capability is for organizations who have implemented the Sanctuary model or other trauma-informed practices throughout their entire organization in a robust manner. Experienced capability is for organizations who have implemented the Sanctuary model or trauma-informed practices throughout much of their organization. Beginner capability is for organizations who have started implementation in a part of their organization and Novice is for organizations learning about the Trauma-informed practices but have not started implementation. - Can volunteer hours count towards meeting the In-Kind contribution of 15%? What about donated goods or materials? Yes. Volunteer hours, donated goods or donated materials can be included as a means to meet the 15% In Kind contribution for a given proposal. In-Kind includes services, material, equipment or labor committed or received at a conservative value that would otherwise be paid from the project budget. Both the donor and the service received should be identified. In other words, in-kind contributions are goods at wholesale value or services provided instead of cash for one of your project budget line-items. Both the revenue and the expense projections should reflect the In-kind contribution. Guidance for how to calculate In-kind:
Professional services (legal, architectural, engineering, accounting, medical) at a customary hourly rate for the Gorge.
Other services (printing, site preparation, fabricating, sub-contractors, publicity) at customary rate for time and materials.
Costs for use of machinery (heavy equipment) at customary hourly rate.
Volunteer time actually involved in project implementation.
Furnishings, food, landscape or construction materials (appliances, furniture, trees, plants, wood, plumbing, hardware etc.) donated by a business or an individual either directly or indirectly.
Hours spent in the actual project (for instance, volunteers constructing a playground), does count as in-kind. Non-skilled volunteer labor such as this would be calculated at the Oregon minimum wage rate.
Labor donated for fundraising activities does not count towards in-kind nor does time in meetings that operate irrespective of a proposal (e.g. Board meetings)
If using volunteer hours, specify the rate used for calculating In-Kind in the Explanation section of the budget form. In Question 11 of the grant application, list names of professional services provided and any other volunteer service valued above minimum wage.
- Can an organization who has billable services still apply? Yes. A proposal that includes one-time or start-up costs for expanding services or other non-covered work would be allowed.
- Question 15 asks about evidenced-based practices, trauma-informed practices or promising practices. If we select yes, what additional information do we provide? If you select yes for Q15, a text box will appear. In the text box indicate which type of practice(s) you are using (evidenced, trauma-informed, promising) and then state which practice(s) specifically will be used. For example, Evidence-based: Nurse Family Partnerships or Trauma-informed: Sanctuary Model.
- What is the electronic submission process? The CGHC is using an online application submission website https://webportalapp.com/appform/columbiagorgehealthcouncil2017 . The link will be available on the CGHC Website starting 8am on Sept 1. Users will need to create an account for each application. An account is an email address and a password. Once an account is created, applicants can complete the necessary information online, save drafts and edit drafts until ready for submission. Once submitted, applications cannot be modified. Specifics on the online submission website are:
- The 16 questions on the Grant application are part of a form that is completed online. You will be able to save drafts and save partially completed forms.
- The Focus Area Checklist is completed online and is incorporated in the Grant Application Form.
- The first and second Letters of Support are 2 separate file uploads. Any additional letters of support need to be combined into a single document as a third upload. You can upload either Word or PDF documents.
- The Goals, Activities and Measures Grid is a separate upload. You can upload either a Word or PDF document.
- The Budget is a separate upload. You can upload either an Excel or PDF document.
- Note: Each application will need a unique applicant account. If your organization is submitting more than 1 application, you will need to create a unique login account for each application.
- It is highly recommended (but not required) that each applicant create the necessary login accounts and save a draft application with Lead Organization information, estimated Total Funding requested and the Focus Area Checklist section completed no later than Sept 15. These specific sections will help confirm that your account is correctly set up and there are no issues with using your account, entering information and saving drafts.
- Can I change my application once submitted? No. You can save your application as a DRAFT and modify as many times as you want. Once you submit an application, you will not be able to edit it further. Please be sure that your application is completed prior to submitting. If you submitted an application by mistake and need to have it returned to a Draft status, send a request to restore the application to draft status to email@example.com. In your request, specify the applicant account it is under as well as the Title. Restoring an application to Draft status is a manual activity and may take up to one day to complete. Once the application is restored to Draft, you will need to revise the application and resubmit prior to the deadline of Sept 22 @ 5pm. Columbia Gorge Health Council staff are not able to edit your application.
