Job Title: Bridges to Health Community Care Coordinator Lead- Full time

The Columbia Gorge Health Council is seeking an individual who is a self-starter to work with a Community Care Coordination Program- Bridges To Health Pathways. This position will work half-time closely with households to help navigate and access community services, health services and other resources and adopt healthy behaviors. The position will work the other half-time with the Bridges to Health HUB staff in supporting their work responding to referrals, gathering data and supporting the Program Manager and HUB Coordinator.

Required: HS Diploma or GED, familiarity with community programs; Must possess a valid driver’s license at time of employment.

Preferred: Successful completion of a Community Health Worker formal training program or equivalent work experience; Bi-lingual English/ Spanish strongly preferred; Experience in a leadership position in the nonprofit, health care or social service sector, complimented with excellent oral and written communication skills; some experience with excel and other Microsoft products.

Salary: $22-$25 per hour plus benefits

Location: Located in Hood River and The Dalles Working remotely from home required during COVID Working from office in Hood River and/or The Dalles after COVID

COVID-19 considerations: During COVID, you will need to work from home. We would supply laptop, etc. Working with clients will require running errands on behalf of clients but not directly meeting with clients unless it is outdoors.
