Community Health Assessment (CHA)
The Regional Columbia Gorge Community Health Assessment is updated every 5 years in collaboration with local hospitals, counties, public health departments, county mental health, and early learning partners. The Community Health Assessment serves as the basis for the next Community Health Improvement Plan, which many community partners use to determine where and how to direct time, money, and resources to improve the overall health of the community.
Please take a moment to fill out our survey, you may take the survey in English or Spanish, and the estimated time to complete is about 10 minutes. Your answers will be kept anonymous. There is an optional chance to win a $100 Visa gift card if you leave your contact information at the end of the survey. The survey will close on July 15th, 2024.
