Getting Connected to Services
Bridges to Health Pathways can help you connect to needed services including health care, dental care, clothing, food, transportation, housing or others.
The goal of the Bridges to Health Pathways Program is to connect you with a Community Care Coordinator to assist you as you take steps toward self-advocacy.
A Community Care Coordinator can:
Meet with you in your home, at an office, or somewhere in your community.
Work with you or your household to partner and build a plan for your family.
Connect you to the services you need most.
Provide you and or your family with support and education.
This is a voluntary program that is of no cost to you
Who is this for: People and families struggling to access resources on their own with two or more needs from the list below Area Served: Living in Hood River or Wasco counties

About Bridges to Health Pathways Program:
Bridges to Health (B2H) is a cross-sector, collaborative approach to providing community care coordination.
The primary goals of B2H are:
Empower community members most in need to improve their overall health and well-being
Improve access to services and resources by addressing disparities in our systems
Increase collaboration of services in and out of healthcare
Bridges to Health is the regional implementation of the Pathways model serving Hood River and Wasco Counties. Pathways is a model that enables a centralized HUB to coordinate, track and measure both the process and the resources to provide distributed community care coordination to people served. Pathways ties payments to milestones that improve client’s health and well-being.

Evolving Systems of Assistance
Bridges to Health employs Community Care Coordinators to help venerable people and populations find and apply for local services, such as Oregon Health Plan, mental health services, transitional housing, and more.
They also collect and qualify their client’s experiences into data to identify gaps or opportunities and feedback that information back to decision & policy makers.
Bridges to Health Evaluation
Bridges to Health Evaluation Metrics
Community Health Workers (CHW’s) are employed by local agencies (hospitals, clinics, schools, social service agencies). They help coordinate needed services for clients & their households. Agencies contract with the Pathways HUB to get paid when evidence-based outcomes are met.

Bridges to Health is administered by Columbia Gorge Health Council, a 501(c)3 non-profit organization.
